Shared Wellness offers a referral bonus for qualified LifeSharing Provider referrals. If you know someone who may be a strong fit for LifeSharing, this program outlines who is eligible, how referrals work, and when bonuses are paid.

Program Purpose

Shared Wellness is committed to recruiting qualified LifeSharing Providers through relationship-based outreach. Because LifeSharing is a highly individualized service model, successful providers bring personal commitment, stability, and strong alignment with our values.

This referral bonus program was created to support that effort by recognizing individuals who help connect Shared Wellness with qualified provider candidates.

Who Can Receive a Referral Bonus?

A referral bonus may be available to individuals who submit a qualified referral, including:

  • Current employees

  • Current LifeSharing Providers

  • Volunteers

  • Community members

  • Family members connected to services

Individuals whose job responsibilities include recruitment, placement, or approval of LifeSharing Providers are not eligible to receive referral bonuses.

Shared Wellness reserves the right to determine eligibility in cases where questions or ambiguity arise.

What Makes a Referral Eligible?

To qualify for the referral bonus program, the referred individual must:

  • Be a new LifeSharing Provider candidate

  • Not have been active or in process with Shared Wellness within the previous 12 months

  • Successfully complete all required screening, training, and approval steps

  • Enter into a LifeSharing Provider Agreement with Shared Wellness

  • Receive a confirmed LifeSharing placement

  • Identify the person who referred them during the application process, according to Shared Wellness procedures

Referrals must be submitted before or at the time of Shared Wellness’s initial contact with the prospective provider. Referrals submitted after a candidate has already initiated contact may not be eligible.

If more than one person refers the same candidate, the referral bonus will be awarded to the first documented referrer.

Referral Bonus Amount

The referral bonus is paid in two installments based on placement and retention milestones.

Initial Placement Bonus — $500

Paid after the referred LifeSharing Provider receives their first confirmed placement and that placement remains active for at least 30 days.

Retention Bonus — $1,000

Paid after the referred LifeSharing Provider has remained actively engaged in LifeSharing services for six consecutive months following placement.

To receive the retention bonus, the provider must be in good standing at the time of review.

If performance, compliance, or quality concerns are identified at the six-month point, Shared Wellness may delay payment until concerns have been addressed and good standing has been restored. Once improvement is demonstrated, the retention bonus may be released at the organization’s discretion.

What Does “Good Standing” Mean?

For the purposes of this program, a LifeSharing Provider is considered to be in good standing when they:

  • Remain in compliance with all contractual, regulatory, and policy requirements

  • Have no substantiated serious safety, compliance, or regulatory violations

  • Demonstrate satisfactory performance and responsiveness to corrective guidance when concerns are identified

All determinations regarding good standing are made solely at the discretion of Shared Wellness.

How Payments Are Made

Referral bonuses are considered taxable income. Payment method is based on the referrer’s relationship to Shared Wellness:

  • Employees: paid through payroll

  • Existing LifeSharing Providers: paid as supplemental compensation with the provider stipend

  • Non-employees: paid through Shared Wellness’s standard payment process

All payments are subject to applicable tax reporting requirements.

Referral bonuses are not paid retroactively.

Program Administration

Participation in the LifeSharing Provider Referral Bonus Program does not guarantee payment. Shared Wellness reserves the right to interpret, modify, suspend, or discontinue the program at any time.

All decisions regarding eligibility, timing, and payment are final and made at the discretion of Shared Wellness.
