Where Clinical Skill Meets

Relationship-Based Care

Support Others. Grow Yourself.

This Warminster opportunity supports an active young man with complex medical needs in a home setting — not an institution.

Our LPNs are trusted professionals who:

• Deliver advanced clinical care
• Collaborate with nurse case management
• Build consistency and stability
• Become a meaningful part of someone’s daily life

Training is provided for trach and ventilator support, and strong oversight ensures you are never practicing in isolation.

If you value both skill and connection — this role was built for you.

Nurses are at the Heart and Health of our Network of Care.

    Whole-Person Care in Action

At Shared Wellness, nursing is at the heart of our whole-person model. Our nurses bridge clinical care, wellness, and everyday living, offering vital checkups, screenings, and wellness education in a setting designed to calm, connect, and support every individual.

    Collaboration at Every Step

Our nurses work hand-in-hand with DSPs, clinicians, families, and program specialists to ensure continuity of care across homes, the Wellness Center, and community settings. Whether coordinating care plans or leading wellness initiatives, they are part of an integrated team that values communication and innovation.
